1. На странице 1 заявления Р11001 заполняем полное и сокращенное наименование ООО. В разделе 2 указываем точный адрес регистрации ООО в соответствии со свидетельством и гарантийным письмом или регистрацией в паспорте (в случае регистрации на домашний адрес руководителя или одного из учредителей). При указании кода субъекта РФ 77 (Москва) или 78 (Санкт-Петербург) пункт 2.4. Город не заполняется.




2. На странице 2 заявления Р11001 завершаем ввод адреса ООО. В разделе 3 указываем сведения о размере уставного капитала ООО.



3. На листе В заявления Р11001 страница 1 вносим следующие сведения об учредителе — физическом лице: фамилию, имя, отчество, ИНН, дату рождения, место рождения и данные документа, удостоверяющего личность (паспорт).

ИНН и отчество при наличии обязательны к заполнению. Если Вы не можете найти свой ИНН или не помните, получали ли Вы его — воспользуйтесь сервисом Узнай свой ИНН. Если ИНН Вы не получали и сервис «Узнай свой ИНН» подтверждает его отсутствие, то оставляете поле ИНН пустым.

В случае если учредителями создаваемого ООО являются два и более физических лица, в отношении каждого такого учредителя заполняется и нумеруется отдельный лист В заявления Р11001. Если учредителем ООО является российское или иностранное юридическое лицо, заполняются листы А или Б соответственно.




4. На листе В заявления Р11001 страница 2 вводим адрес регистрации учредителя. В разделе 7 указываем размер его доли в уставном капитале ООО.



5. На листе Е заявления Р11001 страницы 1 и 2 вносим сведения о руководителе (Генеральном директоре).




6. На листе И заявления Р11001 вносим коды ОКВЭД видов деятельности, которыми собираемся заниматься. Один код должен содержать не менее 4-х цифровых знаков. Дополнительные коды вносятся построчно слева направо.



7. На листе Н заявления Р11001 страница 1 указываем сведения о заявителе. В случае если учредителями создаваемого ООО являются два и более физических лица, в отношении каждого такого учредителя заполняется и нумеруется отдельный лист Н на заявителя. Если лист Н заполняется в отношении единственного учредителя — физ. лица (в разделе 1 проставлено значение 1), раздел 4 не заполняется. В случае 2х и более учредителей в разделе 4 заполняется только пункт 4.1.



8. На Листе Н заявления Р11001 страница 3 указываем порядок выдачи документов. Поля Ф.И.О. и подпись заявителя заполняются только вручную чернилами черного цвета в присутствии налогового инспектора при подаче заявления на государственную регистрацию всеми учредителями ООО. Заверять свои подписи у нотариуса при личной подаче документов на регистрацию ООО не требуется.

Открытие банковских счетов процедура строго регламентированная. В Российской Федерации заявление на счет должно соответствовать форме под номером 0401025. Искать ее не обязательно, все банки Росии уже создали образцы заявлений на открытие счета и свободно выдают их будущим клиентам.

Заявления на открытие банковского счета содержат в себе информацию о:

  • виде регистрируемого счета: расчетный, валютный, депозитный и др.;
  • официальном названии организации или фамилии, имени и отчестве предпринимателя открывающего счет;

  • На первом листе записывается информация о клиенте, его реквизитах и контактах, о должностных лицах, виде и валюте счета. Этот лист заполняется клиентом, его должностными лицами или законными представителями.
  • Второй лист заявления заполняется банком. Здесь указываются реквизиты счета (или нескольких счетов). Информация заверяется печатями и подписями сотрудников Сбербанка.
  • Третий лист представляет собой список приложенных к заявлению документов заверенный клиентом и банком. Это свидетельства, учредительные документы и Устав, приказ о назначении директора и карточка с подписями лиц уполномоченных распоряжаться счетом.

Но воспользоваться средствами, делать платежи и переводы со счета разрешается только после окончания процесса регистрации. Однако этот период редко бывает дольше нескольких дней.

Заявление клиента на открытие счета в банке — это документ, с которого начинаются взаимоотношения кредитной организации с ее клиентом. В нем необходимо указать все свои данные, а также сведения о счете, который нужно открыть. Заполнение и оформление имеют важные нюансы.

Современная жизнь невозможна без совершения банковских операций. И дело не только в том, что нельзя осуществить переводы, получить и отправить переводы или получить заработную плату, но и в том, что другие организации тоже не могут работать без банков. Вследствие этого заявление на открытие текущего счета или другого счета в банке является одним из тех документов, с которыми сталкивается каждый взрослый человек. И не важно, для чего ему требуется сотрудничество с кредитной организацией: для бизнеса или для личных целей.

Правовое регулирование

Заявление на открытие счета в кредитной организации всегда является приложением к договору. Как следует из статьи 846 Гражданского кодекса РФ и Инструкции, утвержденной Банком России от 30.05.2014 № 153-И, он является основанием для открытия счета, тогда как в заявлении необходимо просто указать все персональные данные и приложить к нему требуемые банком документы. Такой порядок является обязательным вне зависимости от того, кому и для каких целей открывается счет:

  • текущий — физическим лицам для совершения личных операций, не связанных с предпринимательской деятельностью или частной практикой;
  • депозитный — для хранения средств как физлиц, так и организаций;
  • расчетный — индивидуальным предпринимателям для ведения предпринимательской деятельности;
  • расчетный — юридическим лицам для расчетно-кассового обслуживания деятельности.

Юридические лица

Банковская карточка обычно оформляется по утвержденной Центробанком форме 0401025, но иногда банки переделывают ее на свое усмотрение, сохраняя основную структуру; пример карточки для открытия расчетного счета показывает, что она состоит из двух блоков:

  • верхний, данные заявителя — заполняет юрлицо;
  • нижний, сведения о предоставленном ему счете — заполняет кредитная организация.

В незаполненном виде образец заявления на открытие счета в банке выглядит так:

Требования к оформлению такие:

  • бланк заполняют от руки с указанием даты заполнения;
  • подписать документ должны руководитель и главный бухгалтер организации. Если должность главбуха не предусмотрена, подписывает только директор;
  • круглая печать ставится при ее наличии. Если организация ее не использует, это требует документального подтверждения.

Заполнение бланка происходит непосредственно в отделении банка, под контролем менеджера, по установленному образцу. Требования в разных кредитных организациях могут отличаться, так что заполнять этот документ заранее на своем бланке не имеет никакого смысла, практически 100%, что придется переписывать на месте.

На второй половине заявления руководитель отделения кредитной организации дает разрешение на открытие юрлицу счета, номер которого вписывается в специальное поле. Если открываются сразу несколько, указывают их все. Затем этот документ хранится в личном деле клиента до окончания действия банковского договора. Кстати, банк потенциальному клиенту вправе и отказать. В этом случае заполненный бланк просто уничтожают, а договор не заключают.

Физические лица

У физических лиц образец заполнения заявления на открытие счета практически не отличается от предусмотренного для организаций, банки вправе изменять и его на свое усмотрение. Кстати, у крупных банков этот бланк можно найти на официальном сайте. Например, у Сбербанка собственное заявление на открытие текущего счета физического лица содержит намного больше информации, чем стандартный бланк. Вот так выглядит его первый лист, а всего их в документе целых три:

Из этой фактически анкеты мы видим, что Сбербанк хочет получить такие персональные данные своего будущего клиента, как:

  • фамилию, имя, отчество;
  • адрес регистрации;
  • почтовый адрес — на него будут приходить официальные письма из банка;
  • контактные телефон и адрес электронной почты;
  • ИНН.

Кроме того, придется указать, какая валюта будет использоваться. А если в банк явился не сам человек лично, а его представитель, то указать данные этого представителя и основания, на которых он действует от имени физлица. Данные должны быть заверены личной подписью гражданина или его представителя с указанием даты заполнения. Остальные страницы анкеты заполняет сам банк, и бланк заявления на открытие депозитного счета для граждан ничем не отличается от того, который заполняется на открытие текущего счета. Но его желательно заполнять прямо в банке, потому что на нем отмечают тип и условия выбранного депозита.

Индивидуальные предприниматели

ИП для банка является тем же физическим лицом, только с правом ведения предпринимательской деятельности: заявление на открытие банковского счета для ИП не имеет никаких особенностей. Он так же, как и юридическое лицо, предоставляет вместе с ним документы о регистрации бизнеса. В настоящий момент это:

  • выписка из ЕГРИП;
  • справка о присвоении ИНН.

Также потребуется паспорт, а в бланке заявления указывается как адрес регистрации, так и почтовый адрес для получения корреспонденции от банка. Заполнение бланка не является гарантией того, что кредитная организация рассмотрит вопрос положительно. Если ИП внесен в черный список лиц, которые подозреваются в финансировании терроризма или отмывании доходов, полученных преступным путем, ему могут отказать в обслуживании. В таком случае банк просто не заключит договор, а заполненное заявление не будет иметь никаких правовых последствий.
